Information for TWLS patients - Omega Loop or Mini Bypass

You have been provided this link after your consultations at Toowoomba Weight Loss Surgery because it is relevant to your upcoming procedure, the costs and the methods of payment.

To applyfor the compassionate release of Superannuation, you will need to gather all the necessary estimates from the relevant providers and upload all these as part of your application.  You will need to contact your Super fund provider about how they will release funds to you upon approval from the ATO.  The ATO provides all the details here. It is then your responsibility to pay each of the providers directly.

It can be overwhelming, so don't hesitate to ask the TWLS team or other patients in the Facebook support group for help if you get stuck in the process of gathering information and making your application.  You can read more about this is our Blog here

To make your application for the compassionate release of super for medical treatment you will need to upload:

​

• Fee estimates from the Surgeon, Assistant, Hospital, Anaesthetist, Dietitian and Psychologist

• Report from Dr Wylie certifying the medical treatment is necessary to alleviate an acute or chronic illness

• Report from your GP accessed here

You will make an online application through your MyGov account attaching your documentation.
